Allowed Amount<\/h4>\n

The maximum dollar amount a provider within your insurance network has agreed to accept for a covered service.<\/p>\n

Balance Billing<\/h4>\n

The additional amount you may be billed if you seek care from a provider that is not within your insurance network.<\/p>\n

Benefit Period<\/h4>\n

The predetermined start and end date of your plan benefits.<\/p>\n

Copayment (copay)<\/h4>\n

The set amount you owe, if any, at the time of the medical service.<\/p>\n

Coinsurance<\/h4>\n

The percentage of the total charges you pay, if any, at the time of service.<\/p>\n

Covered Services<\/h4>\n

Medical services that are eligible for payment under your health plan.<\/p>\n

Deductible<\/h4>\n

The amount, if any, per benefit period, you owe before your insurance company begins to pay for covered services.<\/p>\n

In-network<\/h4>\n

Refers to a provider (person or institution) who is participating in your plan\u2019s network.<\/p>\n

Out-of-network<\/h4>\n

Refers to a provider not in your network, where your out-of-pocket costs will generally be higher.<\/p>\n

Out-of-pocket Maximum<\/h4>\n
