Our practice treats infants and children from birth through adolescence and truly enjoys watching them grow and develop into young adults. Our practice also welcomes children with chronic illnesses and special needs. Our desire is to be their advocate and coordinator of their multi-specialty care. Quality\/ Medical Excellence. All of our pediatricians are board-certified by the American Board of Pediatrics and have trained in some of the best residency programs in the country. Our Certified Pediatric Nurse Practitioners hold a Master\u2019s level or higher degrees in nursing and advanced pediatric training.<\/p>\n
A Medical Home. We want our families to consider us a total resource for their child\u2019s healthcare needs. As your medical home, our doctors and staff use evidence-based medicine and technology to practice the highest quality medical care. If you would like to join our group, please call the office at (904) 387-6200 to schedule a new patient appointment, preferably a well checkup. Please have your insurance card ready when you call.<\/p>\n
With our new Phreesia check-in system, you now have the option of checking in from the convenience of your own home. Several days before your child\u2019s appointment, you will receive an email link and\/or text message that will allow you to \u2018Pre-Register\u2019 for future appointments. Simply click the link, answer a few identifying questions, and you will be able to complete your registration process from home on any smartphone, computer, or tablet.<\/p>\n
Please bring your insurance card to the visit. Parents\/Guardians must bring their Driver\u2019s License. Please bring a copy of your child\u2019s immunization and medical record.<\/p>\n
