Welcome to our Practice.  Thank you for choosing us to participate in your care.  We will provide you with the best quality medical care and support.  We treat our patients like we would like our own family members to be treated.  Don’t hesitate to ask questions or to let us know if there’s something we can do to make you more comfortable.

Please print and complete the new patient paperwork located on this page. Each area must be filled out completely, leaving no blanks at all.  If there is an area that doesn’t apply to you please either write “N/A” next to it or cross through it.  If we receive paperwork that is not complete, we’ll have to return it to you and ask that you fill in every blank.

New Patient Forms 

Once you have completed all of the new patient paperwork, you may either fax it to our office at 386-274-5779, mail it to our office via the US Postal System, or drop it off in person.  We must receive your completed paperwork a minimum of 10 days prior to your scheduled appointment date.  If it is not received in time, your appointment will be canceled and you will need to reschedule to a later date.  Our Medical Assistants enter all of your medical history into your electronic medical record during this time and Dr. Vagovic will review all of your paperwork prior to the day of your appointment.  This reduces the amount of time spent waiting in our office as we make every effort to run on time.  We like to be well prepared for your visit and this is why we require the paperwork in advance.

To schedule an appointment, please call our office and we’ll be happy to schedule you in our first available appointment. Most major insurance carriers are accepted, but we’ll check for you when you call so please have your insurance card available.
