Once you have completed all of the new patient paperwork, you may either fax it to our office at 386-274-5779, mail it to our office via the US Postal System, or drop it off in person. We must receive your completed paperwork a minimum of 10 days prior to your scheduled appointment date. If it is not received in time, your appointment will be canceled and you will need to reschedule to a later date. Our Medical Assistants enter all of your medical history into your electronic medical record during this time and Dr. Vagovic will review all of your paperwork prior to the day of your appointment. This reduces the amount of time spent waiting in our office as we make every effort to run on time. We like to be well prepared for your visit and this is why we require the paperwork in advance.
