To schedule an appointment, please call our office (954) 505-7695, Mondays, Tuesdays, Wednesdays, Thursdays from 9:00am to 4:30pm, and Fridays from 9:00am to 1pm.. Our appointment staff will ask you about the nature of your visit in order to book the most appropriate appointment for you.<\/p>\n
Photo Identification<\/h3>\n
Photo identification is required. A driver\u2019s license, passport or another form of government-issued identification, are all acceptable forms of photo identification. Photo identification must be current. Original photo identification must be presented, photocopies, faxed copies and\/or electronic copies are not allowed.<\/p>\n
Late Cancellation, No Confirmation, No Show<\/h3>\n
We understand that situations arise in which you must cancel your appointment. It is therefore your responsibility to cancel your appointment. You need to provide a notice, the latest 24 hours prior to the visit. The Doctor reserved his time for you that day. Therefore, IF NO CONFIRMATION, LATE CANCELLATION, NO SHOW AFTER CONFIRMATION. Dr. Jose Rivas\u2019s reserves the right to cancel your appointment and a fee of $20 will apply to your account, which is not covered by insurance.<\/p>\n
