Congratulations on your growing family! Welcoming a new baby is an exciting milestone, and we would be honored to be part of this special journey with you.

We encourage expectant parents to schedule a complimentary prenatal visit to tour our office and meet our team, ideally at least four weeks before your due date. To get started, please complete the forms below and email them—along with a copy of your insurance card—to Shorespediatricsinfo@femwell.com. Once we receive your information, a member of the Shores Pediatrics team will reach out to schedule your visit.

Welcome! Whether you’re new to the area or transitioning from another pediatric practice, we’re excited to welcome you to the Shores Pediatrics family.

To schedule your child’s first appointment, please print and complete the forms below and include a copy of your child’s current immunization record. You may email the completed paperwork to Shorespediatricsinfo@femwell.com or fax it to 305-757-8011. After we receive all required documents, a member of our staff will contact you to arrange your first visit.
