For new OBGYN patients, there can be a great deal of paperwork to fill out, but the good news is that we offer an electronic format to make this a very simple process. Once you have scheduled an appointment, you should receive an email from us inviting you to register with our Patient Portal. Once registered, you can complete an online health history form in a secure format that drops right into your chart. We make the process simple, easy and accurate! We will review your entries with you at your visit. If you are unable to complete your history online, we do offer the PDF versions below for you to print out, complete and bring with you to your appointment.
Our new patient center offers education about the kinds of ailments we can treat, our treatment options and more. We can also make the process easier by allowing you to fill out the paperwork here, in our new patient center, anytime before your appointment. This will greatly speed your visit up and allow us more time to focus on your needs.
