Medical Records Release Request
To request your medical records from a previous physician or specialist to be released to Gables Pediatrics, please click here to download our form. Fill out the form and fax it to the appropriate doctor. Please also send us a copy of the form either via fax at 305-441-9110 or email at records@gablespediatrics.com.
If you are requesting a copy of your child’s medical records from our office please email your request to records@gablespediatrics.com. In your email please include the following: your child’s name and date of birth, any specific part of the record requested and for whom the records are being requested (i.e. insurance, specialist, personal). The copied medical records will be available for pick-up in 3 business days. If you need the records sooner than this please contact the office directly. A processing fee for medical records is due at the time of pick-up.

ADHD Assessment (Vanderbilt Assessment Scale)
If your doctor has requested you to complete the Vanderbilt Assessment Scale to evaluate your child for possible ADHD please click here to download the appropriate forms. Please return the completed forms to our office at your earliest convenience. At that time, you will also need to schedule an appointment to discuss the results of the assessment with your doctor.
Policies
Immunization Policy
As a reminder, our physicians strongly believe in and encourage the use of vaccines as recommended by the AAP and CDC. If you are choosing not to vaccinate your child, we suggest looking for an office which more closely shares this philosophy.

Payment Policy
All co-pays, charges and account balances will be due at the time of each of your child’s appointments. We accept all major credit cards, personal checks and cash. If you have any questions regarding charges, previous bills or insurance questions please contact our office at billing@gablespediatrics.com.
Walk-In Policy
Please call to schedule ALL appointments in advance. Walk-in patients will NOT be accepted. Every effort will be made to schedule an appointment for your child as soon as possbile. We appreciate your understanding and patience in this matter.
