Our practice is located on the southeast corner of Mason and Clyde Morris in the blue building.

Appointments & Prescription Refills

To schedule an appointment or refill a prescription please call 386-258-0123

• Most insurance accepted.
• Please make all non-emergency calls and prescription refill calls between during our normal office hours so that your records are readily available.
• Remember to bring your identification and insurance information to your appointment.
• If you are unable to keep your appointment, please notify the office at least 24 hours in advance.

Emergency Care

• In the event of an emergency, go immediately to the emergency room of the nearest hospital and ask them to contact your doctor.
• If the office is open: contact the office and the doctor will be in touch with you, either directly or through office personnel, giving you instructions on where to meet for emergency care.
• If the office is closed: call your doctor’s phone number and the answering service will promptly forward your message to the doctor on-call. You will receive a response as quickly as possible.
