NOTE: If your insurance carrier is not mentioned above and is a PPO/POS plan and/or you are referred to our doctors as specialists from your primary care physician or gynecologist for treatment; we will make every effort to work with you and your insurance carrier to be able to provide medical care once your insurance company approves the services.
